EntityClaimsAI CPT Lookup is a browser-based search aid for a historical workbook identified on the tool page. It lets users search code and description text. It is provided for research, orientation and workflow discussion only.
The tool is not a codebook, coding recommendation, claim-submission decision, reimbursement guarantee, medical diagnosis tool or legal opinion.
2. Ownership and third-party rights
“CPT” and related CPT code-set materials are associated with the American Medical Association (AMA). EntityClaimsAI is not the AMA, does not claim ownership of CPT, and does not imply sponsorship, endorsement or affiliation with the AMA.
ICD materials have separate ownership and publication arrangements. The international ICD classification is maintained by the World Health Organization, while U.S. ICD-10-CM materials are maintained and published through the U.S. Centers for Disease Control and Prevention, National Center for Health Statistics, with applicable government and third-party terms. EntityClaimsAI does not claim ownership of ICD or imply sponsorship or endorsement by WHO, CDC, NCHS or another rights holder.
Other names, payer rules, software names and marks mentioned on this website belong to their respective owners. Their appearance does not create an affiliation.
3. License and permitted use
Access to this website does not transfer ownership of any code set, description, workbook, compilation, documentation or other protected material to you.
You may use the lookup interface for lawful internal reference and research consistent with the rights applicable to your organization. You must not:
- represent EntityClaimsAI as the owner, publisher or official authority for CPT or ICD;
- copy, scrape, mirror, sell, sublicense or redistribute the dataset or substantial portions of it unless you have the required rights;
- use search results as the sole basis for coding, billing, reimbursement or compliance decisions; or
- remove notices, attribution or technical safeguards.
Your organization is responsible for confirming that its use of any displayed data is properly licensed. If you do not have the required rights, do not download, reproduce or redistribute the data.
4. Accuracy and currency
The current tool is labelled as a historical 2021 reference. Codes, descriptions, edits, clinical conventions, payer policies and local regulatory requirements may change. Data may contain omissions or errors and has not been independently certified by EntityClaimsAI.
Before coding, billing, submitting a claim or making a compliance decision, consult the current authorized code-set materials, payer instructions, applicable Oman requirements and a qualified coding or compliance professional.
5. No professional advice or guarantee
Information from this tool is general information only. EntityClaimsAI does not provide medical, coding, reimbursement, accounting or legal advice through the tool. No result guarantees coverage, payment, claim acceptance or a particular reimbursement outcome.
6. Privacy and security
Do not enter patient names, identifiers, diagnoses, dates of birth, insurance numbers or other protected health information into search fields or contact forms. Search terms may be visible in your browser and network logs.
The tool is intended for code lookup, not patient-data processing. Use your organization’s approved systems and privacy procedures for any patient information.
7. Availability and changes
The tool is provided on an “as available” basis. EntityClaimsAI may update, limit, suspend or remove the tool, dataset, features or this notice without guaranteeing uninterrupted access.
Use of the tool after a notice update means you acknowledge the updated notice. If you need clarification about a permitted use, contact EntityClaimsAI before copying or distributing any data.
